This report discusses the pilot integrated healthcare delivery network that was implemented in Hennepin County, Minnesota. The project targeted childless adults between the ages of 18 and 64 who were at or below 75 percent of the Federal Poverty Guidelines. Patient needs were attended to by integrating medical, behavior, and human services in an effort to reduce the occurrence of expensive health crises. In the first year of the program, costs were reduced between 40 percent and 95 percent for the costliest participants. Healthcare savings are being investing in a new sobering center, hiring more staff, and purchasing housing units for its homeless participants. The pilot project was authorized in Chapter 1, Article 16, Section 20 of Minnesota's 2010 First Special Session.
